（利用規約）

この利用規約（以下、「本規約」といいます。）は、EtonHouseがサマープログラムに関して提供するサービスの利用条件を定めるものです。サマープログラムにお申し込みされる皆さまには、本規約に従って、このサービスをご利用いただきます。

 

参加時の年齢と異なる年齢枠で申し込まれた場合は、自動的に受け付け取り消しとなります。異なる年齢枠で申し込まれた事が確認され次第、自動的にお申し込みは取り消しされ、ご登録のEメールアドレスに取り消しされた旨が連絡されます。

 

クレジット決済のご請求は株式会社JIA（日本でイートンハウスを運営する会社）からとなり、クレジットカード決済の請求名は イートンハウスやEtonHouse或いはその他の表記となることを理解しました。（表記はお使いのクレジット会社で決められる形となります）

 

【EtonHouseからのメール連絡】サマープログラムに関するご案内は、全て【フォームにご登録のEメールアドレス】に【@etonhouse.co.jp】のドメインから送信されることとします。

 

【受信設定】【@etonhouse.co.jp】のドメインからEメールが受信できるよう設定することに同意するものとします。

 

【Eメールのご登録】携帯のメールアドレスは登録出来兼ねることを理解しました。

 

【受け入れの判断】以下の事項に同意し、理解します：

1) 行動面・学習面等において懸念がある場合は、申込用紙にその詳細を正確に記入すること。
2) 申込用紙に申告の記載がない、または申告内容が不十分な状態で参加した場合、EtonHouseは参加資格を取り消す権利を有し、その場合は返金は行われないこと。
3) 事前のアセスメントに協力すること。
4) 参加の可否はEtonHouseが判断し、その決定に対して異議を申し立てないこと。事前のアセスメントで参加不可と判断された場合には、参加費は全額返金されるものとする。

 

【宿泊と移動(交通)】以下を理解し、同意します。

(1)宿泊先と移動(交通)は各ご家庭で手配する必要があること
(2)宿泊と移動(交通)は料金に含まれないこと

 

【保護者による毎日の送迎】毎日保護者による会場までの送迎が必要であり、EtonHouseでは送迎サービスのご提供はしていないことを理解しました。

 

【守秘義務】 本フォームにてお申し込みの内容は秘密情報とし、EtonHouseは秘密情報に関して他の保護者や第三者に開示しないことを理解しました。

 

【写真及び動画の掲載】本規約に同意した時点で、参加者及び保護者その他の関係する者は、当プログラム開催期間中にプログラム会場で、当プログラムスタッフ又は業務委託先が参加者等を撮影した写真又は動画について、EtonHouseが広告等一切の媒体に自由に利用できることに同意したものとみなします。なお、保護者が参加者等の写真または動画の使用を希望されない場合は、office@etonhouse.co.jpまでご連絡の上、専用フォームを指定期日までにご提出いただくものとします。

 

【プライバシー】以下を理解し、同意します。

(1)EtonHouseは他の参加者のプライバシーに関する情報を開示しないこと

(2)以下のご質問にはお答えしかねること
(A) 友達が申し込んだかどうか
(B) 友達がどの週に申し込んだか

 

【キャンセル規約】以下を理解しました

2026年5月10日以前の(5月10日も含む)のキャンセル料：

キャンセルする週の合計額の10%

 

2026年5月11日以降のキャンセル料：

返金不可（いかなる理由でもご返金不可となります）

 

【キャンセル】 

以下を理解しました：

(1)キャンセルはCancellation Form のみの受付であること（Eメールや電話でのご連絡は不可。キャンセルフォームのみでの受付）

(2) Cancellation Formを送信すると、すぐ処理されお申し込みされた席は埋まってしまうこと

(3) キャンセルをした後にやはり参加したいとなった場合、改めて再度申し込みフォームに記入し提出し直す必要があること

 

【お申し込み週の変更】日程変更はできません。

申し込んだ週をEtonHouseが変更できかねることを理解し、同意します。変更を希望する場合は、Cancellation formに入力・送信し、ご希望の週で再度予約をし直すことに同意します。

 

【お申し込みの譲渡や変更】

お申し込みは、他者、翌年、或いは他のプログラムに変更不可であることを理解しました。

 

【サマープログラムのキャンセル】

参加人数が少なく催行が難しいと判断した場合はEtonHouseがお申し込みの週をキャンセルすることがあることを理解しました。その際はお申し込み者に連絡をして返金の手続きをします。

 

【ランチの持参】

糖尿病をお持ちの参加者はランチを毎日持参することに同意します。

 

【忘れ物に関して】

忘れ物に関し、以下を理解しました。

(1)忘れ物はご自宅への郵送が出来かねること (着払いも不可）
(2) 忘れ物の受け取りは、事前にEメールにて連絡する必要であること
(3) 1ヶ月以上連絡がない場合は、EtonHouseが保護者に通知なく破棄すること

 

【お持ち物に関して】

万が一、紛失、破損、盗難があった場合にはEtonHouseは一切責任を負わないことを理解します。

 

【全てのナッツの持ち込み禁止】

全てのナッツ類（栗やココナッツを含む）やナッツを含む食品（ピーナッツバターを含む）のお持ち込み不可。

 

天災（地震、台風等）によりプログラムを提供できない場合の振替や補償は一切行われないことを理解しました。

I understand and agree to the following Terms and Conditions.

 

If registered in the incorrect age group, your sign-up will be canceled automatically. EtonHouse will cancel your sign-up and inform you of the cancellation via email to the registered email address.

 

I understand that the Credit Card Payment is billed by JIA Co., Ltd (A company that runs EtonHouse in Japan) and that the name that will appear on my credit card is イートンハウス, EtonHouse or others (The name is decided by each customer’s credit card company).

 

【Email from EtonHouse】I understand all the Emails regarding the summer program will be sent to the 【Email address I registered on the sign up form】 from the @etonhouse.co.jp domain.

 

【Email Setting】 I agree to change my Email setting in a way that can receive Emails from the @etonhouse.co.jp domain.

 

【Email Registration】I understand that I cannot register a mobile Email address.

 

【Assessment】I understand and agree to the following:

1) If there are any concerns regarding behavior, learning, or related matters, I agree to provide accurate and detailed information on the sign-up form.
2) If no relevant information is provided on the sign-up form, or if the submitted information is insufficient, EtonHouse reserves the right to revoke the participant’s eligibility. In such cases, no refund will be issued.
3) I agree to cooperate with any required pre-program assessments.
4) The decision regarding participation will be made by EtonHouse, and I agree not to dispute this decision. If the participant is determined to be ineligible based on the pre-program assessment, the full participation fee will be refunded.

 

【Confidentiality】I understand the following: All information collected during registration remains confidential and EtonHouse is not required to disclose this information to the parents or any other person if it is not relevant or appropriate to do so.

 

【Consent for Use of Photos and Videos】While the program is in operation, the EtonHouse program staff as well as the vendor contracted to record the program may be taking pictures (including video photography). By registering for the program, it is understood that the participant, parent/guardian and anyone associated with those that may be at the program venue consent to being photographed, and to allow EtonHouse to use these materials in any medium for publicity purposes. If a parent/guardian does not consent to the use of photographs or videos of the participant, they must contact office@etonhouse.co.jp and submit the designated form by the specified date.

 

【Privacy】I understand the following: 

(1) EtonHouse does not disclose information related to other participants’ privacy
(2) EtonHouse does not answer the following questions:

(A) Whether my friend has signed up
(B) Which week my friend has signed up

 

【Accommodation & Transportation】I understand the following:

 (1) Parents are required to book accommodations by themselves 

(2) This program does not provide or include the following: accommodation & transportation　

 

【Daily Drop off & Pick up by Parents】I understand that my child must be dropped off and picked up daily during the program and that there is no transportation service.

 

【Cancellation Policy】 I understand the following:

Cancelling on or before May 10th, 2026:

Cancellation fee: 10% of the total amount of week(s) cancelled

 

Cancelling on or after May 11th, 2026:

No refund (for any reason)

 

【Cancellation】I understand the following:

 (1) A Cancellation request must be made by filling out a 【Cancellation Form】 (No Email or Phone Call)

(2) Upon receipt of the Cancellation Form, my spot is canceled immediately and that the spot will be filled 

(3) After sending a Cancellation Form, if I happen to change my mind and decide to join, I need to fill out the Registration form again and start the registration process from the start.

 

【Changing the sign-up week】

I understand and agree that EtonHouse is unable to make changes to the weeks I have signed up for. If I were to change my registered week, I agree to fill out the cancellation form and sign up again for my preferred week.

 

【Sign-ups are not transferable】

I understand that sign-ups are not transferable to other parties, years, or programs.

 

【Cancellation of program】

In the event that EtonHouse is unable to acquire a certain number of enrollments, I understand that EtonHouse reserves the right to cancel corresponding weeks of the summer program. In that case, EtonHouse will contact the family and provide a refund.

 

【Bringing Lunch】

As a participant managing diabetes, I understand and agree to provide my own lunch daily.

 

【Items that are left behind】

I understand regarding my child’s belongings that are left behind:

(1) EtonHouse will not be able to send it to my house
(2) In order to pick up the items, I need to contact EtonHouse via Email prior to picking it up
(3) When belongings are left behind more than 1 month, EtonHouse will dispose of the item without notifying parents

 

【Items brought to the program】

I understand that EtonHouse is not responsible for any missing, lost, or stolen items brought to the program.

 

【Nut free School】

I understand that the school is a nut free school and that all the nuts (including chestnuts, coconuts, etc) and any products that contain nuts (including peanut butter, almond butter etc) cannot be brought into the school.

 

I understand that in the event EtonHouse is unable to provide the program due to a natural disaster (e.g., earthquake, typhoon, etc.), EtonHouse will not be able to offer makeup lessons on alternative dates or provide any form of compensation.
